Street Suite 140, Anchorage<\/li>\n<\/ul>\n<p>Providers and facilities will continue working with their local child care assistance office regarding eligibility for the CCAP to include submission of<br \/>\n    rate changes.<\/p>\n<p>The <em>Request for Payment<\/em> CC78 form will be revised effective July 1, 2019 and available on the CCPO website at: <a href=\"http:\/\/dhss.alaska.gov\/dpa\/Pages\/ccare\/forms.aspx\">http:\/\/dhss.alaska.gov\/dpa\/Pages\/ccare\/forms.aspx<\/a>    <\/p>\n<p>Please be sure you are using the most current version of this form.
